Agency Referrals
We welcome our community partners, medical practices and hospitals to send direct referrals for services for their patients. We are pleased to offer therapy for clients with existing medication providers, or medication management services for individuals with existing therapy services.
To refer a patient, please complete and upload the referral form provided to you by our office. Please allow us 2-3 business days to process the referral and reach the client. If you would like a copy of our referral form, please contact our office at (315) 391-7770. If you intend to collaborate on patient care, please provide a release of information with the referral.
